To request a copy of your New York Proton Center medical records, submit a request online using your computer, tablet, or smartphone.
How to Request Your Records
- Gather the necessary information, including your name, date of birth, address, the specific records and date range requested, and your preferred delivery method.
- Click here to complete the secure online request form.
- Review your information carefully before submitting the form.
- Complete the DocuSign authorization sent to your email. We cannot process your request until the authorization is signed.
